Please make your reservations between 8:30am and 4:00pm (Monday through Friday) and provide at least 48 hours notice for in-county trips and 72 hours for out-of-county trips. Middle Keys Transportation also has an answering machine available for you to leave a message, 24 hours a day, 7 days a week. When leaving a message please include the following information:

  • Name
  • Date & Time of Appointment
  • Will you need a return trip?
  • Pick up Location
  • Nature of Appointment (i.e. Medical, Dental…)
  • Doctors name, address and phone number
  • Phone Number where you can be reached
  • Are you in a wheelchair?
  • Will you have an escort, service animal or medical equipment (oxygen)?

If you’re a first time caller please also include:

  • Date of Birth
  • Social Security Number
  • Medicaid Number (if applicable)
